What does a child at risk for drug abuse look like?

Although we don’t want to admit it, many of us have an image of an at-risk adolescent. This image may reveal any number of racial, socioeconomic, or gender stereotypes about which kids are most likely to use drugs. The stereotypes may be very damaging- especially when they cause parents to believe that their own children aren’t at risk, because they are.

Dr. Sandra Wills Hannon, health communications expert, explains the danger of these kinds of stereotypes. "Some parents may think, 'My child doesn't fit the profile of a drug user, so I don’t need to talk to him or her about drugs.'" Other parents might think, 'We are a member of this or that at-risk group, so no matter what I do, my child will probably use drugs.'

Dr. Wills Hannon explains that both of these inferences are mistaken. "All parents need to know that their adolescents—simply by virtue of being adolescents—are at risk for drug abuse. This does not mean that all adolescents use drugs. In fact, most don't. But adolescence is a time when all parents can help their children establish lifelong patterns of healthy behavior. If a person reaches the age of 21 without ever trying drugs or alcohol, the odds are very small that they will ever have a drug or alcohol problem."

Teen Drug Abuse and Drug Testing Kits

The Effects of Media on Teen Drug Abuse

Would you allow a complete stranger to come into your home and spend time with your children? Of course not. Yet, many parents allow their children to spend large amounts of unsupervised time watching television, surfing the Web, or listening to popular music.

While there are many good and educational television programs, Web sites, and popular songs, there are also many that contain messages harmful to youth—messages about drugs, alcohol, body image, sex, and violence—and kids often take these messages to heart, especially when they come from some of their favorite celebrities.

According to the American Academy of Pediatrics, parents can help their children by watching and listening to the messages their children receive. This critical viewing skill is called media literacy. There are many ways parents can help their kids become media literate. Some suggestions:

  • Watch television, surf the Web, and listen to music with your child. Find out what kinds of media they are being exposed to. Ask probing questions about why your child finds these shows, Web sites, and songs appealing. Have them look or listen for pro-drug or other unhealthy messages and discuss how to resist those behaviors.
  • Remind your child that there are people—actors, producers, directors, screenwriters, songwriters, corporate sponsors, advertising executives, and others—whose job it is to create these messages to inform or entertain. Discuss any messages about drugs that are untrue or that show drug use in a positive way. Point out the disadvantages of using drugs, like failure in school or losing the trust of the family.
  • Help your child look for media messages that might not be so obvious. Ask questions like, "Does this song suggest that violence is a good way to solve problems?" "According to this movie, what are women like? What are men like?" "What does this commercial say will happen if you wear those jeans? Is that true?" "Is drug use portrayed as positive or glamorous?"
  • Use movies, advertising, or TV programs that show drinking alcohol or smoking cigarettes as a catalyst to discuss why people drink or smoke and why the behavior was perhaps not necessary to the scene.

Effects of Drug Abuse on Learning

Drugs erode the self-discipline and motivation necessary for learning. Pervasive drug use among students creates a climate in the schools that is destructive to learning.

Research shows that drug use can cause a decline in academic performance. This has been found to be true for students who excelled in school prior to drug use as well as for those with academic or behavioral problems prior to use. According to one study, students using marijuana were twice as likely to average D's and F's as other students. The decline in grades often reverses when drug use is stopped.

Drug use is closely tied to being truant and dropping out of school. High school seniors who are heavy drug users are more than three times as likely to skip school as nonusers. About one-fifth of heavy users skipped three or more school
days a month, more than six times the truancy rate of nonusers. In a Philadelphia study, dropouts were almost twice as likely to be frequent drug users as were high school graduates; four in five dropouts used drugs regularly.

Drug use is associated with crime and misconduct that disrupt the maintenance of an orderly and safe school atmosphere conducive to learning. Drugs not only transform schools into marketplaces for dope deals, they also lead to the destruction of property and to classroom disorder. Among high school seniors, heavy drug users were more than three times as likely to vandalize school property and twice as likely to have been involved in a fight at school or at work as nonusers. Students on drugs create a climate of apathy, disruption, and disrespect for others. For example, among teenage callers to a national cocaine hotline, 32 percent reported that they sold drugs, and 64 percent said that they stole from family, friends, or employers to buy drugs. A drug-ridden environment is a strong deterrent to learning not only for drug users butfor other students as well.

Drug Abuse & Pre Employment Drug Testing

Employee Drug Testing & Drug Testing Kits

A drug test will have to be taken by anyone selected to work with the former Emergency Medical Services of Trinidad and Tobago (EMSTT) under its new management.

This will also apply to the paramedics and other workers now employed in the service.

Replacing the EMSTT will be Global Medical Response of Trinidad and Tobago, a joint venture between Amalgamated Security Services Ltd and Trinidad Global Medical Services, based in Miami, Florida, USA.

The takeover becomes effective on October 1.

The EMS has since 2003 been administered by the South West Regional Health Authority for the Ministry of Health, under the management of Joanne Salazar.
Current EMS employees have been invited to re-apply between September 5 and 9 for work with the new ambulance services company.

"But before they, or any other set of workers, are given a contract they will have to undergo a drug test," a senior GMRTT director said.

Privatisation of the service was reported exclusively by the Daily Express in January.

And last month GMRTT was awarded a $150 million contract to run it for the next three years.

Johnny Aboud, of Amalgamated Security, said yesterday that "the focus of the company will be to provide a proper ambulance service". At present 283 technical people are working with the EMSTT to manage 37 ambulances stationed in 14 locations.

The ambulance services in Tobago are run by the Tobago Regional Health Authority.

Workers employed with EMSTT are reportedly wary about the new package offered by GMRTT.

The workers said they were not "at liberty" to speak to the media on the matter and any questions should be referred to the Public Services Association.

Senior officials of the PSA were not in office yesterday but it was learned that the new salary offered to the EMSTT workers was more than they were receiving at present.

The basic monthly salary of a technician is $3,900 plus $250 meal and laundry allowance.

GMRTT is offering the employees a flat $4,800 plus medical insurance and a savings plan.

The technicians now work an average of 168 hours, but they will be expected to work between 192 and 200 hours a month under their new contract.

Aboud said the company held a meeting last Thursday at which the employees were told that they will have the option of working more hours and would be paid accordingly. The company started as the Emergency Health Services in 1999 as a pilot programme under the management of a company from Nova Scotia, Canada.

Employee Drug Testing Programs & Drug Treatment

Employee drug testing is unfortunately one of the most controversial and troublesome issues facing American employers. Rising employee turnover, absenteeism, tardiness, on-the-job accidents, health benefit utilization and costs, workers compensation claims, inventory shrinkage and employee theft, lowered productivity, and workplace violence are problems directly attributable to substance abuse. Employer concern and action are equally warranted these problems cost to the U.S. workplace well over $110 billion each and every year.

To address the illicit drug problem, employers in the U.S. have implemented substance abuse policies and instituted drug and alcohol testing programs to screen out substance-abusing applicants and to detect substance abusers on the job. Employee drug testing programs help protect the health and safety of all employees, and mitigate the costs associated with having drug abusers on the payroll. They provide early identification, and the ability to refer employees with substance abuse problems for treatment. The integration of drug screening procedures with appropriate drug education and treatment programs is proving not only to be an effective way of managing substance abuse, but also a valuable tool in achieving positive employee relations, delivering significant cost savings, and providing corporations with a competitive advantage.

The Americans With Disabilities Act allows employers to conduct testing for substance abuse or the use of illegal drugs, and some states provide voluntary or legislated guidelines for drug and alcohol testing. Regulations in most states deny unemployment and/or workers compensation benefits for employees that test positive for illicit drug use. And, most workers compensation insurers allow insurance premium discounts for corporations that have instituted employee drug testing programs.

Additional items to consider relative to employee drug testing:

  • The federal Drug-Free Workplace Act requires federal government contractors and grantees to certify that they are providing a drug-free workplace. The Act applies to all contracts or grants that equal or exceed $25,000.
  • If you do not test, you may find yourself flooded with applications of workers who were afraid to apply for jobs elsewhere because of other employers’ use of pre-employment testing.
  • The majority of current employees generally do not object to the screening of applicants or employees.
  • Many employers test all employees on a random basis, and all applicants, while some employers limit testing to those employees whose drug use may endanger themselves, co-workers, or the general public, or for instances of “reasonable suspicion”, “ return-to-duty”, or “post-accident”. NOTE: Limiting testing only to employees with previous drug-related convictions or to those within certain geographic or demographic categories may be contrary to federal, state, and local anti-discrimination laws prohibiting arbitrary treatment of persons within protected classes.

Drug Abuse & Many Different Home Drug Testing Kits

Drug Abuse in Different Ages, Races & Drug Testing

The National Survey on Drug Use and Health (NSDUH) obtains information on nine different categories of illicit drug use: marijuana, cocaine, heroin, hallucinogens, inhalants, and nonmedical use of prescription-type pain relievers, tranquilizers, stimulants, and sedatives. In these categories, hashish is included with marijuana, and crack is considered a form of cocaine. Several drugs are grouped under the hallucinogens category, including LSD, PCP, peyote, mescaline, mushrooms, and "Ecstasy" (MDMA). Inhalants include a variety of substances, such as amyl nitrite, cleaning fluids, gasoline, paint, and glue. The four categories of prescription-type drugs (pain relievers, tranquilizers, stimulants, and sedatives) cover numerous drugs available through prescriptions and sometimes illegally "on the street." Methamphetamine is considered a type of stimulant. Respondents are asked to report only uses of drugs that were not prescribed for them or drugs they took only for the experience or feeling they caused. Over-the-counter drugs and legitimate uses of prescription drugs are not included. NSDUH reports combine the four prescription-type drug groups into a category referred to as "any psychotherapeutics."

Estimates of "any illicit drug use" reported from NSDUH reflect use of any of the nine substance categories listed above. Use of alcohol and tobacco products, while illegal for youths, are not included in these estimates.

Prevalence, by Type of Drug. In 2003, an estimated 19.5 million Americans aged 12 or older were current illicit drug users, meaning they had used an illicit drug during the month prior to the survey interview. This estimate represents 8.2 percent of the population aged 12 years old or older. There was no change in the overall rate of illicit drug use between 2002 and 2003. In 2002, there were an estimated 19.5 million illicit drug users (8.3 percent).

Marijuana is the most commonly used illicit drug, with 14.6 million current users (6.2 percent of the population) in 2003. An estimated 2.3 million persons (1.0 percent) were current cocaine users, 604,000 of whom used crack (0.3 percent). Hallucinogens were used by 1.0 million persons (0.4 percent). There were an estimated 119,000 current heroin users (0.1 percent). All of these estimates are similar to estimates for 2002.

The number of current users of Ecstasy decreased between 2002 and 2003, from 676,000 (0.3 percent) to 470,000 (0.2 percent). Although there were no significant changes in the past month use of other hallucinogens, there were significant declines in past year use of LSD (from 1 million to 558,000) and in past year overall hallucinogen use (from 4.7 million to 3.9 million) between 2002 and 2003, as well as in past year use of Ecstasy (from 3.2 million to 2.1 million).

Of the 8.8 million current users of illicit drugs other than marijuana in 2003, 6.3 million were current users of psychotherapeutic drugs. This represents 2.7 percent of the population aged 12 or older. Of those who reported current use of any psychotherapeutics, 4.7 million used pain relievers, 1.8 million used tranquilizers, 1.2 million used stimulants, and 0.3 million used sedatives. These estimates are all similar to the corresponding estimates for 2002.


Pregnant Women. Among pregnant women aged 15 to 44 years, 4.3 percent reported using illicit drugs in the month prior to their interview during 2002 and 2003. This rate was significantly lower than the rate among women aged 15 to 44 who were not pregnant (10.4 percent). (These estimates are based on combined 2002 and 2003 NSDUH data.)

Race/Ethnicity. Rates of current illicit drug use varied significantly among the major racial/ethnic groups in 2003. The rate of illicit drug use was highest among American Indians or Alaska Natives (12.1 percent), persons reporting two or more races (12.0 percent), and Native Hawaiians or Other Pacific Islanders (11.1 percent). Rates were 8.7 percent for blacks, 8.3 percent for whites, and 8.0 percent for Hispanics. Asians had the lowest rate of current illicit drug use at 3.8 percent. There were no statistically significant changes between 2002 and 2003 in the rates of current illicit drug use for any racial/ethnic group.


Employment. Current employment status was highly correlated with rates of current illicit drug use in 2003. An estimated 18.2 percent of unemployed adults aged 18 or older were current illicit drug users compared with 7.9 percent of those employed full time and 10.7 percent of those employed part time. Although the rate of drug use was higher among unemployed persons compared with those from other employment groups, most drug users were employed. Of the 16.7 million illicit drug users aged 18 or older in 2003, 12.4 million (74.3 percent) were employed either full or part time.

Frequency of Use. In 2003, 12.2 percent of past year marijuana users used marijuana on 300 or more days in the past 12 months. This translates into 3.1 million persons using marijuana on a daily or almost daily basis over a 12-month period. This was the same number as in 2002. However, the number of youths aged 12 to 17 using marijuana daily or almost daily declined from 358,000 in 2002 to 282,000 in 2003. The number of youths using marijuana on 20 or more days in the past month declined from 603,000 in 2002 to 482,000 in 2003.

Driving Under the Influence of Illicit Drugs. In 2003, an estimated 10.9 million persons reported driving under the influence of an illicit drug during the past year. This corresponds to 4.6 percent of the population aged 12 or older. The rates were 14.1 percent among young adults aged 18 to 25 and 3.1 percent among adults aged 26 or older. These rates were all similar to the 2002 rates.

The drugs students are taking today are more potent, more dangerous, and more addictive than ever.

Adolescents are particularly vulnerable to the effects of drugs. Drugs threaten normal development in a number of ways:

* Drugs can interfere with memory, sensation, and perception. They distort experiences and cause a loss of self-control that can lead users to harm themselves and others.

* Drugs interfere with the brain's ability to take in, sort, and synthesize information. As a result, sensory information runs together, providing new sensations while blocking normal ability to understand the information received.

* Drugs can have an insidious effect on perception; for example, cocaine and amphetamines often give users a false sense of functioning at their best while on the drug.

Drug suppliers have responded to the increasing demand for drugs by developing new strains, producing reprocessed purified drugs, and using underground laboratories to create more powerful forms of illegal drugs. Consequently, users are exposed to heightened or unknown levels of risk.

* The marijuana produced today is from 5 to 20 times stronger than that available as recently as 10 years ago. Regular use by adolescents has been associated with an "amotivational syndrome," characterized by apathy and loss of goals. Research has shown that severe psychological damage, including paranoia and psychosis, can occur when marijuana contains 2 percent THC, its major psychoactive ingredient. Since the early 1980s, most marijuana has contained from 4 to 6 percent THC-two or three times the amount capable of causing serious damage.

* Crack is a purified and highly addictive form of cocaine.

* Phencyclidine (PCP), first developed as an animal tranquilizer, has unpredictable and often violent effects. Often children do not even know that they are using this drug when PCP-laced parsley in cigarette form is passed off as marijuana, or when PCP in crystal form is sold as lysergic acid (LSD).

* Some of the "designer" drugs, slight chemical variation of existing illegal drugs, have been known to cause permanent brain damage with a single dose.

Drugs cause physical and emotional dependence. Users may develop a craving for specific drugs, and their bodies may respond to the presence of drugs in ways that lead to increased drug use.

* Regular users of drugs develop tolerance, a need to take larger doses to get the same initial effect. They may respond by combining drugs, frequently with devastating results. Many teenage drug users calling a national cocaine hotline report that they take other drugs just to counteract the unpleasant effects of cocaine.

* Certain drugs, such as opiates, barbituates, alcohol, and nicotine, create physical dependence. With prolonged use, these drugs become part of the body chemistry. When a regular user stops taking the drug, the body experiences the physiological trauma known as withdrawal.

* Psychological dependence occurs when taking drugs becomes the center of the user's life. Among children, psychological dependence erodes school performance and can destroy ties to family and friends, as well as cause the child to abandon outside interests, values, and goals. The child goes from taking drugs to feel good, to taking them to keep from feeling bad. Over time, drug use itself heightens the bad feelings and can leave the use suicidal. More than half of all adolescent suicides are drug-related.

* Drugs can remain in the body long after use has stopped. The extent to which a drug is retained in the body depend on the drug's chemical omposition, that is, whether it is fat-soluble. Fat-soluble drugs such as marijuana and phencyclidine (PCP) seek out and settle in the fatty tissues. As a result, they build up in the fatty parts of the body such as the brain. Such accumulations of drugs and their slow release over time may have effects on the mind and body weeks or even months after drug use has stopped.

Alcohol Drug Test and Drug Abuse

Drug use is widespread among American schoolchildren. Although a national study of high school seniors in 1991 shows that drug use among young people is declining, the figures remain unacceptably high (see Figure 2). The United States continues to have the highest rate of teenage drug use of any nation in the industrialized world. Forty-four percent of high school seniors have tried an illicit drug by the time they graduate. Alcohol is the most widely used drug. By their senior year, 88 percent of students in the class of 1991 had used alcohol; 78 percent had used alcohol in the past year and 54 percent had used it in the month prior to the survey. Thirty percent of seniors surveyed reported at least one occasion of heavy drinking in the two weeks prior to the survey--an occasion in which they had five or more drinks in a row. Twenty-four percent of 1991 seniors reported using marijuana in the past year, and 14 percent said they had used it at least once in the previous month. Three and one-half percent of seniors indicated that they had used cocaine in the past year. Three percent of seniors had used crack, and 1.5 percent had used it within the last year.

The drug problem affects all types of students. All regions and all types of communities show high levels of drug use. Thirty percent of 1990 high school seniors in nonmetropolitan areas reported illicit drug use in the previous year, while the rate for seniors in large metropolitan areas was 33 percent. Although higher proportions of males are involved in illicit drug use, especially heavy drug use, the gap between the sexes is closing. The extent to which high school seniors reported having used illicit drugs is higher for whites than for blacks.

Initial use of alcohol and other drugs occurs at an increasingly early age. Nineteen percent of seniors report they had initiated cigarette use by sixth grade and 11 percent had used alcohol. Forty-four percent of 8th graders have tried cigarettes, and 70 percent have at least tried alcohol. Twenty-seven percent of 8th graders have gotten drunk at least once, and 13 percent report they have consumed five or more drinks in a row. Of the illicit drugs, marijuana and inhalants show the earliest pattern of initiation; about 2.8 percent of seniors had begun using both of these substances by the 6th grade. The peak initiation rate is reached by 9th grade. Peak initiation rates for cocaine and hallucinogens are reached in 10th and 11th grade with the initiation rate for nearly all drugs falling off by 12th grade.

The National Survey on Drug Use and Health (NSDUH) includes a series of questions to assess dependence on and abuse of substances, including alcohol and illicit drugs, which include nonmedical use of prescription-type drugs. These questions are designed to measure dependence and abuse based on criteria specified in the Diagnostic and Statistical Manual of Mental Disorders, 4th edition (DSM-IV) (American Psychiatric Association [APA], 1994). The questions on dependence ask about health and emotional problems, attempts to cut down on use, tolerance, withdrawal, and other symptoms associated with substances used. The questions on abuse ask about problems at work, home, and school; problems with family or friends; physical danger; and trouble with the law due to substance use. The survey also asks about the receipt of treatment for problems related to substance use. Specialty treatment is treatment received at drug or alcohol rehabilitation facilities (inpatient or outpatient), hospitals (inpatient only), or mental health centers. An individual is defined as needing treatment for an alcohol or drug problem if he or she was dependent on or abused alcohol or drugs or received specialty treatment for alcohol or drugs in the past 12 months.

Dependence or Abuse, by Type of Substance. An estimated 21.6 million persons aged 12 or older in 2003 were classified with substance dependence or abuse (9.1 percent of the total population). Of these, 3.1 million were classified with dependence on or abuse of both alcohol and illicit drugs, 3.8 million were dependent on or abused illicit drugs but not alcohol, and 14.8 million were dependent on or abused alcohol but not illicit drugs. Of the 6.8 million persons
classified with dependence on or abuse of illicit drugs, 4.2 million were dependent on or abused marijuana, 1.5 million were dependent on or abused cocaine, and 1.4 million were dependent on or abused pain relievers. Between 2002 and 2003, there was no change in the number of persons with substance dependence or abuse (22.0 million in 2002 and 21.6 million in 2003).

Dependence or Abuse, by Age. In 2003, the rate of substance dependence or abuse was 8.9 percent for youths aged 12 to 17; 21.0 percent for persons aged 18 to 25; and 7.0 percent for persons aged 26 or older. Among persons with substance dependence or abuse, illicit drugs accounted for 58.1 percent of youths, 37.2 percent of persons aged 18 to 25, and 24.1 percent of persons aged 26 or older.

Dependence or Abuse, by Gender. In 2003, males were almost twice as likely to be classified with substance dependence or abuse as females (12.2 vs. 6.2 percent). Among youths aged 12 to 17, however, the rate of substance dependence or abuse among females (9.1 percent) was similar to the rate among males (8.7 percent).

Dependence or Abuse, by Race/Ethnicity. The rate of substance dependence or abuse was highest among American Indians or Alaska Natives (17.2 percent). The next highest rates were among Native Hawaiians or Other Pacific Islanders (12.9 percent) and persons reporting two or more races (1
