Hemoglobin A1c-At-Home Test Instructions
1) Fill out Form

Fill out form completely and legibly. An incomplete form may prevent the laboratory from testing. IMPORTANT: You will need a doctor's authorization for this test. If you desire, our medical director will authorize the test for you. If you would like your doctor to receive the results, please fill out his/her name and address on the Mailing Label provided. You MUST fill in the date that you applied the blood to the collection circles. NOTE: Patients who live in Maryland or Nevada are required to get authorization from their local doctor. |
2) Wash and Dry Hands

Thoroughly wash your hands with soap and water. Completely dry your hands. |
3) Prick Finger
|
Prick your finger using the enclosed lancing device or you can use your own lancet. |
4) Drop Blood onto Collection Circles
|
Squeeze blood from your finger to fall onto each of the collection circles. The blood should completely fill each circle. Overfill is acceptable. |
5) Dry Overnight
Place your form in a clean dry place to dry overnight. Do not cover. Do not expose to direct heat or sunlight.
6) Place Finished Form in Plastic Bag
You MUST fill in the date that you applied the blood to the collection circles. Remove and keep the pink copy. Place the rest of the form with the dried blood samples into the plastic bag. Seal the bag well.
7) Fill Out Mailing Label IF
- Your doctor signed the Test Request Form. - Or our medical director authorizes the test and you want your doctor to receive a copy of the test results.
8) Place Label and Sealed Plastic Bag in Return Envelope
Place the Label (if required) and the bag containing the form with the dried blood samples in the Return Envelope.
9) Mail Return Envelope
Place a first class postage stamp on the Return Envelope provided and mail before the last mail pickup. IMPORTANT - Do not allow to remain in the mailbox over the weekend. Store at room temperature.
