• This test is confidential. No names or personal information is ever used in the testing process. Instead a PDT-90 ® code number which is supplied in the kit is used to reference the hair sample and to receive the results confidentially and anonymously over the phone. • Test results are available typically 5 working days from the time that they are received at the lab, provided that all of the instructions for submitting the sample are followed properly. To receive the results, simply call a toll-free results hotline.
   
• Hair samples are cleansed to remove any external contaminants. As with any drug testing, cutoff levels are utilized to help ensure that inadvertent or unintentional contact with drugs will not create a false positive result.
   
• The testing technology is designed to show the presence of drug levels which would only be consistent with the ingestion of drugs. All samples are tested using a radioimmunoassay screen. Before any positive test result is reported, the sample is subjected to a second, confirmatory test using gas chromatography/mass spectrometry (GC/MS), considered the gold standard for forensic drug testing.
   
• How does the PDT-90 Drug Testing Service compare with urinalysis tests currently on the market?
  Urinalysis can only check to see if drugs were used within the previous 2-3 day period for most drugs, longer for chronic marijuana use. PDT-90 testing can check for drug use over several months period.
   
• Results from this test may not be admissible as evidence in a legal situation.
   
• Hair must be cut directly from the donor's head or body, it is not advisable to use a hair sample taken from a hairbrush or from any other source/surface.

Important: The hair sample must be taken directly from the donor's scalp or body. It is not advisable that a hair sample is taken from a hairbrush or any other source (e.g. pillow or shower drain) as this sample may be contaminated and it will not be possible for the laboratory to determine to root ends of the sample. Therefore the sample will be rejected by the laboratory.

If no head hair is available, body hair can be used. Bleaches, shampoos and external contaminants (i.e. marijuana smoke) have no known impact on test results.

INSTRUCTIONS

STEP 1: READ ALL OF THE INSTRUCTIONS CAREFULLY It is important that you read and understand all of the materials for completing the PDT-90 Personal Drug Testing Service™ drug test with hair. If you do not follow the instructions properly, we may not be able to process your drug test and provide you with results.

STEP 2: COLLECT THE SAMPLE Take the hair sample directly from your child's head. We advise against taking samples from other sources, such as hair brushes, since there is no guarantee that the sample is from your child.

1. Remove the materials contained inside the Hair Sample Collection Package. Your package should contain a white Sample Acquisition Card, a red Integrity Seal and foil.
2. Fold the foil in half lengthwise and open, making a trough, and place aside in preparation for the collection.
3. Use a sharp pair of blunt-nosed scissors to collect the hair. (Scissors not included.)
4. Position yourself behind the child in order to take the sample from the back "crown" of the head.
5. Find the area of the hair that is fullest and lift to expose the lower layer of hair.
6. Grasp a small lock of hair visibly equal to 1/2 inch wide and about one strand deep when held flat across your finger. (Photo #1.) Note: The drug test with hair cannot test sample unless we have a sufficient quantity of hair.
7. Cut hair as close to the scalp as possible.

1. Put scissors down and grasp cut end firmly. Keeping the cut ends aligned, place the hair sample into the foil with the cut ends extending 1/4 inch beyond the slanted end of the foil (Photo #2).
2. Press the sides of the foil together, trapping the hair sample tightly inside. If hair is long, wrap the remaining hair around the foil (Photo #3). DO NOT CUT THE ENDS OFF THE HAIR!
3. NEVER cut hair that is in a braid. Undo the braid before you collect the sample. If the hair is short or curly it may help to wrap the foil around the hair prior to cutting. This will ensure proper alignment.
4. Insert the foil-wrapped sample into the Sample Acquisition Card with the root ends to the left.
5. Tear the PDT-90 Code Card from the Sample Acquisition Card. The Code Card contains your PDT-90 code number and our toll-free results number. Place it in a safe place, since you will need it to receive the drug test hair results.
6. Place the Integrity Seal on the Sample Acquisition Card once the sample has been inserted.
7. Date the Integrity Seal on the Sample Acquisition Card.
8. Read and initial the Sample Acquisition Card in the space provided.
9. Please retain your PDT-90 Code Card which contains your testing code number and the Psychemedics 800#.

STEP 3: MAIL THE COLLECTED SAMPLE

1. Insert the Sample Acquisition Card into the first class postage-paid return envelope.
2. Seal the envelope and drop into any mailbox. No postage is needed.

STEP 4: CALL TOLL-FREE FOR RESULTS Provided that the collection instructions are followed properly:

Once we receive the collected sample, our laboratory will test the sample for the presence of drugs such as cocaine, meth, marijuana, heroin, ecstasy, pcp, crack, thc, and opiates.

To receive the drug test results, call the provided toll free number with your PDT-90 code number. Test results should be ready 5 business days after receipt at the lab, but they may take longer.
